What you'll do

• Provide spiritual care to clients and families
• coordinate referrals and home or facility visits
• provide counseling and bereavement follow-up
• engage clergy and community groups to address spiritual needs
• facilitate memorial services when appropriate
• contribute to staff education, patient care conferences, quality improvement, and community bereavement programs
• document care and follow regulatory and organizational standards.
