What you'll do

• Train and guide Direct Support Professionals on program plans and documentation
• support staff coaching and compliance
• advocate for residents’ rights and preferences
• monitor progress and adjust programming
• collaborate with families and care teams
• develop and monitor Individual Program Plans
• maintain accurate documentation and reports
• lead plan meetings and reviews
• coordinate with regional center and healthcare partners
• support audits and licensing visits.
