What you'll do

• Conduct routine patient check-ins

• coordinate appointments and monitoring schedules
• gather patient information and prepare charts or notes
• track required labs and monitoring
• route prescription-related requests and renewals to pharmacists
• document care and organize follow-up workflows
• maintain confidentiality and participate in training and process improvement.
