Community Health Worker

What you'll do

• Conduct patient outreach and follow-up calls
• assist with Medicaid applications and benefits enrollment
• coordinate referrals to community resources
• follow up on patient needs after discharge
• collaborate with emergency department and inpatient teams, social workers, care managers, and community partners
• travel between Aspire locations as needed.
