What you'll do
- Develops and implements discharge planning within an interdisciplinary healthcare team.
- Addresses complex clinical care needs by identifying and removing barriers that prevent optimum access to needed post-acute care.
- Works collaboratively with providers to identify discharge barriers and delays to optimize efficient use of acute care hospital days and prevent prolonged length of stay.
- Provides clinical leadership to healthcare teams and ensures daily goals are met according to the patient’s plan of care.
