Social Care Navigator

Job overview

The Social Care Navigator role is onsite in Derby, NY; compensation is $19–$21 per hour. This part-time position is approximately 18–22 hours per week and supports Harmonia Collaborative Care’s work connecting community members with health and well-being resources. The navigator screens for social-care needs, connects people with services, and follows up on referrals. The role helps individuals address barriers to support through accurate documentation and communication with clients, providers, and community partners.
