The Rehabilitation Specialist – Psychosocial Day Program is an onsite, full-time position in Fairfax, VA, with an annual salary of $48,000–$54,000. The position is based at Hope House and supports HopeLink Behavioral Health’s psychosocial programs. HopeLink provides behavioral health, crisis intervention and suicide intervention services. The role exists to help program members build skills, manage symptoms and move toward greater community integration. Through person-centered rehabilitation, psychoeducation, crisis support and care coordination, the specialist supports clients’ recovery, wellness and participation in work, education or volunteering.

What you'll do

• Develop person-centered Recovery/Transition Plans
• provide rehabilitative skill-building in the day program and community
• deliver psychoeducation to individuals and groups
• support independent living, interpersonal and professional skills
• provide training related to co-occurring substance use and mental illness
• assist with volunteer and supported education placements
• provide crisis support as needed
• complete clinical documentation
• coordinate care with providers and case managers
• screen for additional support needs
• and promote client leadership and advocacy.
