What you'll do

• Complete and update service coordination assessments
• coordinate person-directed planning and individual plans of care
• facilitate planning meetings, make community referrals, identify barriers, and advocate for service needs
• monitor participant progress and maintain contact with participants, families, and others involved in plans
• complete required documentation and billing
• meet face-to-face and collateral contact expectations
• coordinate with agency departments and community partners.
