Part-Time Family Physician – Remote

What you'll do

• Provide virtual care to Medicare patients with chronic, complex, and overlapping medical, behavioral, and social needs
• collaborate with nurse practitioners using internal care-delivery technology and clinical judgment
• support care coordination
• supervise nurse practitioners providing multidisciplinary care
• help develop and refine clinical programs
• collaborate with physicians, care managers, and other care team members.
