Skyline Ridge Nursing & Rehabilitation Center is a member of the Stellar Senior Living community. This role assists licensed Physical Therapists in providing therapy services to help residents improve mobility, strength, and overall function, contributing to their rehabilitation goals.

What you'll do

• Implement therapy treatment plans designed by the supervising Physical Therapist.
• Guide residents through exercises and activities to improve mobility, strength, balance, and functional independence.
• Monitor residents’ progress during therapy sessions and report updates to the supervising Physical Therapist.
• Educate residents and their families on exercises and techniques to continue therapy goals outside of sessions.
• Accurately document therapy sessions, residents’ progress, and outcomes in compliance with facility and regulatory requirements.
• Collaborate with interdisciplinary team members to ensure a holistic approach to resident care.
• Provide therapy services on an as-needed PRN basis to support residents’ care plans.
