The Home Health Physical Therapist (PT) is an onsite role based in Vista, CA, serving the East County San Diego region. Compensation is up to $135 per visit. This position supports the Lifespan Therapy team in providing care to homebound geriatric patients. This role exists to conduct comprehensive in-home evaluations and implement therapeutic interventions to improve patient mobility, functional strength, and independence.
What you'll do
- Conduct comprehensive in-home evaluations
- Develop individualized plans of care for geriatric patients
- Focus on functional strengthening, gait training, balance, and fall risk reduction
- Manage caseload and conduct home visits
- Collaborate with a supportive care team
- Document patient notes using the TherapySync EMR system.
