What you'll do

• Provide annual physicals, preventive screenings, acute visits, and chronic disease management
• assess patients, diagnose conditions, develop treatment plans, prescribe medications, and interpret tests
• counsel patients in English and Spanish
• coordinate referrals and follow-up
• maintain timely EHR documentation
• collaborate with clinical and administrative staff.
