Community Habilitation Specialist

What you'll do

• Teach daily living skills such as cooking, budgeting, and using public transportation
• help individuals access community resources and activities
• follow and adapt support to Individual Service Plans
• document progress and complete required records
• promote safety and individual rights
• communicate with families and coordinators
• transport individuals in a personal vehicle as needed, with mileage reimbursement.
