Family Support Specialist II

The specialist works with a multidisciplinary team supporting residents through a Choice Neighborhoods Implementation grant.

What you'll do

• Conduct resident intake and assessments
• make home, office, and community visits
• develop and monitor family development plans
• connect residents with social service organizations and public agencies
• advocate for residents and coordinate assigned caseload services
• maintain confidential records and enter progress data in LEARN
• attend partner and program meetings
• and complete related administrative reporting and billing tasks. Occasional evening, weekend, and travel availability is required.
