What you'll do

• Assist clients and families in behavioral health recovery and wellness
• support recovery coaches and clinical teams in implementing service plans
• help members navigate community resources
• make referrals and follow up with agencies
• document services
• communicate client functioning and support needs to the clinical team
• provide crisis support as needed and notify the supervisor or team
• assist with appointments, benefits, legal consents, and transportation
• support discharge planning and welfare checks
• and coordinate physical and behavioral health services.
