What you'll do

• Complete patient databases, treatment plans, progress notes, and discharge summaries
• orient patients to program objectives, requirements, rules, rights, and services
• assess and place patients according to treatment phases
• maintain clinical documentation and charts
• counsel patients in crisis and regarding program compliance
• arrange community referrals
• review treatment plans
• participate in case conferences
• complete intakes and facilitate group counseling
• collect and test urine or swab drug screens using chain of custody
• submit reports and attend supervision and team meetings.
