Social Services Designee

Job overview

The Social Services Designee role is full-time and onsite in Minneapolis, MN, paying $21.25–$27.50 per hour. The designee coordinates admissions and discharges, provides resident and family support, and connects facility staff with community agencies. What you'll do

• Manage admissions, referrals, screenings, paperwork, financial applications, and discharge planning
• coordinate home health or equipment needs
• provide emotional support and information to residents and families
• act as liaison among residents, families, staff, and community agencies
• complete social assessments and care-plan documentation
• participate in interdisciplinary care planning and conferences
• maintain records and follow facility policies and regulatory requirements.
