Contracted Case Manager

What you'll do

• Identify individual needs and preferences
• locate resources and make service referrals
• prevent duplicated services
• maintain contact with providers and manage payment authorizations
• develop and update annual support plans
• promote health, safety, independence, and self-advocacy
• complete at least two in-person visits annually
• document work in the electronic record system.
