• Conduct initial assessments and reassessments
• evaluate client needs and program eligibility
• develop service plans within consumer-choice and budget guidelines
• partner with vendors to implement services
• monitor changes in client needs
• assist with problem solving and referrals
• support public-benefit and health-insurance applications
• coordinate safe hospital and nursing-facility discharges
• conduct consumer-directed care skills training
• maintain timely records
• and participate in rotating office coverage.
