What you'll do

• Execute community education training and ongoing support for Senior Life Solution programs
• train teams in referral development, public speaking, advocacy messaging, community engagement, and hospital integration
• develop and monitor community education plans
• identify community and referral-source needs
• coordinate outreach, events, and partnerships
• track referral performance and use data to improve outcomes
• support compliance, quality improvement, and strategic planning
• provide mental health and health-related education to community groups as requested.
