What you'll do

• Accept member handoffs and review care plans
• serve as ongoing contact for members and authorized supports
• conduct planned outreach
• monitor service access and changes affecting stability
• address manageable barriers
• make limited maintenance referrals
• arrange return to active navigation when needed
• document contacts and outcomes
• coordinate with families, providers, and community partners
• elevate service gaps.
