The Licensed Practical Nurse (LPN) Visit Nurse – Hospice, PRN role is onsite across Pekin, East Peoria, Washington, and surrounding areas in Illinois, with pay of $28–$31 per hour. The nurse supports Transitions Hospice’s interdisciplinary care team and patients and families who need additional care.

What you'll do

• Provide nursing care within scope of practice
• assess patients and families and contribute to individualized care plans
• complete visits as needed
• administer medications and treatments per physician orders
• document clinical notes, including pain and symptom management outcomes
• communicate changes in condition to physicians and obtain orders as required
• coordinate with community health partners
• arrange equipment and services
• and participate in continuing education and in-service training. Local travel is required.
