Job overview
The Functional Wellness and Rehabilitation Educator role is remote, with the candidate required to reside in California and travel regionally up to 75% during new center launches. Compensation is $141,000–$161,000 per year, plus a target annual bonus of 10% and possible additional incentive compensation. Habitat Health provides coordinated medical and social care to older adults through its PACE model. The educator leads onboarding and training for assigned roles at PACE Centers, supporting consistent competencies, workflows, and best practices. The role combines direct instruction with process improvement to support participant care, operational quality, compliance, and team engagement.
What you'll do
- Oversee training for assigned roles, including physical therapists, occupational therapists, activities coordinators, and personal care attendants
- lead on-site learning for existing centers and new launches
- coordinate training, practicum, and shadowing
- oversee competency signoffs, skills validation, and preceptor checklists
- visit assigned centers
- partner with the central Learning and Development team
- develop and improve workflows, standard operating procedures, playbooks, job aids, and educational resources
- review processes and recommend improvements.
