Adult Care Coordinator

Job overview

The Adult Care Coordinator role is onsite in Niagara Falls, NY, and includes on-call responsibilities. Compensation is $24.70 per hour. What you'll do

• Provide direct care-management contacts
• maintain ongoing contact with consumers and families and work toward service-plan goals
• conduct intake interviews and assessments
• prepare case documentation and administrative reports
• provide supportive contact, counseling, advocacy, and information
• link consumers to services and coordinate hospital admissions and discharges
• provide after-hours on-call assistance
• maintain relationships with referral sources and service agencies
• coordinate with billing staff on Medicaid status and billing.

Why this role

Requires an associate’s or bachelor’s degree in social work or a related field, 1–2 years of specialized training and experience with the SPMI or mentally disabled population, and a valid New York State driver’s license. Use of a personal vehicle during work hours is required.
