What you'll do

• Evaluate mental health and complex care concerns
• assess clinical evaluations and treatment appropriateness
• identify social and environmental factors
• coordinate with case managers, recovery care coordinators, providers, commands, and families
• facilitate referrals and access to medical and mental health resources
• update records and monitor care-plan progress.
