The Day Center Certified Nurse Aide (CNA), Weekdays 8am–4:30pm role is onsite in Holland, MI, with pay of $18.36–$19.87 per hour. The CNA supports LifeCircles PACE’s adult day program for participants aged 55 and older. The role provides person-centered, hands-on support that helps participants maintain independence, dignity, and quality of life. Working with the interdisciplinary team, the CNA contributes to participants’ safety, comfort, and well-being.

What you'll do

• Assist with bathing, grooming, dressing, and toileting according to each participant’s Life Plan
• support meals and hydration
• assist with mobility, positioning, and safe transfers
• measure and record vital signs, weight, and intake/output
• report condition changes to the interdisciplinary team and leadership
• maintain confidentiality and professional conduct.
