What you'll do

• Consult and communicate care objectives with dentists, specialists, dental laboratories, and community resources
• assess patient and family healthcare, education, and psychosocial needs
• maintain referral and community-resource relationships
• triage and resolve patient concerns
• support referrals and insurance pre-authorizations
• coordinate individualized care plans and monitor responses
• manage incoming and outgoing referral communications
• maintain case-management documentation and database records
• review engagement and communication practices
• collaborate with emergency departments to reduce repeat visits for dental conditions
• support complex patients’ timely access to care and treatment-plan follow-through.
