Memory Care Coordinator

What you'll do

• Coordinate daily memory care operations
• help develop and update resident care plans
• monitor changes in residents’ condition, behavior, abilities, or needs
• communicate updates to families, providers, nurses, and leadership
• support residents experiencing dementia-related behaviors
• model redirection and person-centered care
• supervise and coach caregivers, CNAs, and medication technicians
• assist with training
• coordinate with other departments
• participate in assessments, admissions, care conferences, and family meetings
• maintain regulatory compliance and confidentiality.
