Medical Case Manager

Job overview

What you'll do

• Provide client-centered case management and supportive services
• develop and monitor individualized service plans
• coordinate medical, behavioral health, housing, and other resources
• conduct client meetings and visits
• maintain a caseload of approximately 50–55 clients
• document services and outcomes
• support Ryan White compliance and quality improvement
• and collaborate with multidisciplinary teams and community partners.
