What you'll do

• Provide direct support, instruction, encouragement, and assistance
• help with mobility, dressing, bathing, toileting, and personal hygiene
• schedule appointments and outings
• coordinate and provide transportation
• maintain active and inactive medical records according to established policies and procedures
• document care and follow support plans.
