The role supports patient recovery through evidence-based and holistic care in collaboration with a multidisciplinary treatment team and patients’ families.

What you'll do

• Conduct comprehensive personal, medical, and psychosocial assessments
• develop individualized treatment and rehabilitation plans
• provide individual and group counseling
• educate patients and families about substance use disorders, co-occurring conditions, and community resources
• advocate for patient needs and facilitate access to services
• maintain case histories, treatment plans, and progress notes
• evaluate and modify plans
• use motivational interviewing and other therapeutic techniques
• promote abstinence and educate about relapse risks.
