Hospice Liaison

Coordinates and facilitates referrals to Hospice care based on Hospice admission criteria. Assists in information gathering, education, and form completion for patient intake, admissions, and transitions to home. Develops consultative relationships and makes effective customer contacts and presentations. Develops and implements marketing and education programs to increase awareness of the company.
