What you'll do

• Conduct initial and ongoing assessments

• educate patients and families
• provide emotional support and crisis intervention as needed
• manage an assigned caseload
• coordinate with hospice team members
• document care in the electronic medical record
• comply with hospice regulations
• participate in the on-call rotation and respond to after-hours patient needs.
