What you'll do

• Complete Community Health Worker services certification
• support patients in following provider instructions
• assess patient and family information needs related to health, mental health, and social services
• provide service information, education, and referrals
• liaise among patients, families, behavioral health and human service providers
• work with multidisciplinary teams on action plans and service connections
• connect patients to support groups and provide emotional and educational support
• maintain precise records
• participate in continuing and in-service education, peer support, quality improvement, performance review, and advocacy activities.
