What you'll do

• Assess current, new, and prospective residents’ psychosocial needs, strengths, and limitations
• communicate with residents, families, and case managers
• build supportive relationships
• coordinate care with interdisciplinary teams
• develop and update individualized psychosocial care plans
• participate in care planning meetings
• provide counseling, emotional support, and resource referrals
• maintain compliant documentation.

Why this role
