What you'll do

• Build trusting relationships and support clients with housing, financial, vocational, educational, legal, and medical needs
• make referrals and coordinate services
• complete functional assessments, crisis plans, and Individual Community Support Plans in compliance with Minnesota Rule 79
• provide crisis support
• meet with clients to monitor progress and adjust plans
• develop community relationships and advocate for clients.
