What you'll do

• Assist with daily living activities and personal care as needed
• support independent living, social, and community integration skills
• assist with hygiene, meal preparation, and medication reminders
• encourage recreational, educational, vocational, and community activities
• maintain documentation and reports
• monitor health, safety, and behavioral needs and report concerns
• transport individuals to appointments, activities, and community outings as required
• promote dignity, choice, and person-centered support.
