What you'll do

• Build relationships with hospitals, physicians, discharge planners, rehabilitation centers, assisted living communities, elder law attorneys, senior centers, and other referral sources
• represent the organization at community events and educational programs
• educate families and partners about home care and geriatric care management
• support care coordination, long-term care planning, advocacy, and transitions
• track referral activity and growth metrics
• contribute to operational improvement, quality assurance, compliance, and strategic planning.
