Hospice Registered Nurse (RN) Care Manager

The Hospice Registered Nurse (RN) Care Manager role is onsite and field-based in Marysville, OH, serving Marysville, Plain City, London, Jefferson, and surrounding communities. Compensation is not specified. The schedule is Monday–Friday, 8:00 AM–4:30 PM, with a holiday and weekend rotation. The role supports Ohio's Hospice’s interdisciplinary hospice team. This role provides skilled, compassionate nursing care to patients in their homes and care facilities. It contributes to patient comfort and support by assessing needs, coordinating individualized care with the interdisciplinary team, and guiding patients, families, and caregivers through the hospice experience.

What you'll do

• Provide patient-centered nursing care in homes and care facilities
• assess patients and families and develop individualized care plans
• monitor condition changes and adjust medications, interventions, plans, and visit frequency as needed
• communicate with physicians and the hospice team
• coordinate interdisciplinary care
• educate and support patients, families, and caregivers
• advocate for patients
• document care in the electronic health record
• participate in admissions, team meetings, care conferences, and quality improvement
• support and mentor team members as needed.
