What you'll do

• Conduct assessments and develop individualized service plans
• monitor progress and provide coaching and accountability
• connect participants with housing, healthcare, education, employment, and other resources
• provide job readiness and placement support
• facilitate workshops and support groups
• collaborate with families, employers, landlords, community organizations, and justice-system partners
• maintain participant records, data, and reports
• support confidentiality and compliance.
