What you'll do

• Coordinate and provide safe, timely, effective, equitable, and patient-centered care
• manage case assignments and service plans
• review progress and determine case closure
• communicate patient needs and follow up with care team members
• manage discharge plans
• collaborate with patients, families, therapists, and referral sources
• maintain comprehensive clinical records
• advocate regarding treatment options, insurance, and financial assistance
• monitor progress and revise plans as needed
• coordinate with social services, health, and government agencies
• facilitate group therapy and document group notes.
