What you'll do

• Develop and grow referral relationships throughout the assigned South Las Vegas territory
• build partnerships with physicians, hospitals, skilled nursing facilities, discharge planners, case managers, and other healthcare providers
• conduct face-to-face outreach
• identify referral opportunities and strategies to increase patient census
• educate referral partners about home health services
• maintain existing accounts and develop new business
• track referral activity, market trends, and sales results using CRM and reporting tools
• collaborate with clinical and operational teams
• represent the organization at community and healthcare networking events.
