The Licensed Nursing Assistant (LNA) — Hospice House, Friday–Sunday Nights role is onsite in Concord, NH, with a Friday–Sunday schedule from 7:00 p.m. to 7:00 a.m. Compensation is $26–$28 per hour, inclusive of weekend and overnight differentials. The role supports Granite VNA’s Hospice House team. The LNA provides daily assistance and comfort to residents while supporting their dignity and quality of life. The role follows individualized care plans, observes and reports changes, and communicates with residents, families, and the clinical team.

What you'll do

• Assist residents with bathing, grooming, eating, mobility, transfers, and incontinence care
• follow individualized care plans
• observe, document, and report changes in residents’ conditions
• maintain a safe, clean, comfortable environment
• support residents with cognitive needs and meaningful activities
• communicate respectfully with residents, families, nurses, and care team members
• follow agency policies and LNA scope of practice.
