What you'll do

• Assess patients and collaborate with the care team to develop and implement plans of care
• refer patients to other disciplines based on identified needs or document the rationale for not doing so
• teach patients self-care and promote health and independence
• supervise LPNs and HHAs
• complete accurate and timely documentation
• communicate patient progress to the clinical manager and care team
• plan and provide staff education
• participate in clinical development and continuing education
• and perform assigned on-call responsibilities and services.
