What you'll do

• Conduct nutritional assessments and develop individualized meal plans
• collaborate with healthcare teams to incorporate nutrition goals into care plans
• educate and counsel residents and families on nutrition and wellness
• assist with menu planning, food safety, and quality control
• maintain resident nutrition records and progress
• participate in performance improvement activities.
