What you'll do

• Provide dependable support every Sunday for scheduled church attendance
• assist with transportation to and from church as authorized
• provide appropriate assistance before, during, and after services
• promote independence, dignity, personal choice, and community participation
• follow individualized care and safety plans
• communicate with the participant and care coordination team
• complete required documentation and Electronic Visit Verification.
