External Marketer — Skilled Nursing Facility Referrals

What you'll do

• Build and maintain relationships with hospital case managers, discharge planners, social workers, care coordinators, physicians, home health, hospice, and community partners
• generate, track, and follow up on referrals
• educate referral sources about services, payer acceptance, and admissions criteria
• conduct in-person sales and market visits
• monitor census, payer mix, and referral trends
• maintain referral and census reports
• partner with admissions, administrators, DONs, and corporate leadership.
