What you'll do
- Coordinate development and completion of resident assessments
- maintain and update MDS and care-plan policies and procedures
- assist residents with the care-plan portion of discharge planning
- coordinate new-admission notifications with the Director of Nursing Services
- help schedule assessment and care-plan meetings
- assist with treatment and activity selection based on residents’ medical and social histories
- participate in quarterly and annual reviews.
