The MDS Coordinator – Licensed Practical Nurse (LPN) role is onsite in Albuquerque, NM, in a post-acute setting. Pay is $30–$42 per hour. The role supports the facility’s interdisciplinary team in completing and validating resident assessments and care plans.

What you'll do

• Coordinate Minimum Data Set (MDS) assessments with the interdisciplinary team
• schedule assessment reference dates and coordinate completion of assessment sections and care area assessments
• ensure timely transmission of assessments
• review validation reports and facilitate corrections
• maintain reports
• support resident care plan updates
• identify significant changes in resident status
• participate in staff education and quality improvement
• and compile MDS data reports.

Why this role
