What you'll do

• Provide hands-on patient care at the assigned home, facility, or adult family home
• follow individualized care plans and report changes or observed needs to the nurse supervisor
• assist with activities of daily living, including grooming, bathing, oral hygiene, feeding, toileting, transfers, ambulation, and range of motion
• support skin integrity through repositioning and related care
• perform delegated tasks such as taking vital signs and weights and applying creams or ointments
• follow safety and infection-control procedures
• communicate with residents, families, visitors, peers, and supervisors
• participate in the on-call caregiver rotation.
